Job Description
Job Summary:
The Licensed Practical Nurse - Home Care/Hospice provides basic clinical care to clinic patients under the direction of a registered nurse or physician, functioning within scope of license.
Responsibilities:
+ Contributes to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).
+ Administers medications and observes patients for adverse reactions to medications or treatments.
+ Performs routine laboratory tests and simple therapeutic services.
+ Educates patients/families about disease treatment plan including self care post discharge, holistic health needs, available resources and follow up care.
Qualifications
Licenses/Certifications/Registration:
+ Required Credential(s):
+ CertifiedBLS Providercredentialed from theAmerican Heart Association (AHA)obtained within1 Month (30 days)of hire date or job transfer date.
+ LicensedDrivers License.
+ LicensedPractical/Vocational Nursecredentialed from theNew York State Education Department Office of Professionsobtained within2 Weeks (14 days)of hire date or job transfer date.
Education & Work Experience:
+ Trade School
+ Graduate of an accredited practical or vocational Nursing Program.
+ Knowledge of Allscripts preferred.Work history in primary care preferred.
+ Experience with administering immunizations preferred.
+ Experience in hospice and/ or home health nursing preferred.
+ Must have excellent verbal skills and written skills- position requires ability to multi-task, remain positive and calm in high stress situations and possess solid customer service skills.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
