4K a year 8 days ago 8 days ago 8 days ago The Pavilion at Vestal Park is seeking a full-time Case Manager for our Assisted Living Facility.
Primary hours:
Monday - Friday day shift; Participates in night/weekend on-call rotation The Case Manager will be responsible for:
Initial and periodic evaluation, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs; Orientating a new resident and family to the daily routine and assisting each resident to adjust to life in the facility; Assisting each resident to maintain family and community ties and to develop new ones; Arranging for services from public and private sources for income, health, mental health and social services; Assisting the resident in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual's health and mental health needs; Assisting residents in need of alternative living arrangements to make and execute sound discharge or transfer plans Job Requirements:
Clinical Experience Must be at least 21 years of age Be of good moral character, as evidenced by at least three recent letters of recommendation from individuals knowledgeable of the proposed administrator's background, qualifications and interest in the field of adult care Have the following combination of education and experience, per regulations:
A case manager shall have the following education and work
Experience:
A bachelor's or master's degree from an accredited college or university with major work in human resources or service delivery, and one year of full-time experience in the provision of services to a dependent adult population or an associate degree from an accredited college or university with major work in human resources or service delivery, and three years of full-time experience in the provision of services to a dependent adult population.
